Helpful Information
Please be aware that email will be handled in a
non-urgent manner.
Please allow 48-72 hours for a response to your email. We will not accept appointment or prescription requests in email.
Need a refill on your prescription? Please call your pharmacy 1-2 weeks before you run out to request they fax a refill request to allow us time to refill it. Dr. Nayar may be away or unavailable to respond immediately to your urgent requests.
FORMS: Please allow 2 weeks for forms to be completed by Dr. Nayar. If able, email a copy to him so that he can determine if he needs to see you to fill out sections of the form. You can email to dr.nayar3155@gmail.com (be aware you will get a bounce back message but rest assured your message went through). If forms are to be dropped off, please fill in any patient information required before dropping them off. Some forms will require a visit to our office or phone call with Dr. Nayar and there may be a charge for filling it in.
